Fat transplantation involves removing fat from areas such as the buttocks or abdomen and injecting it into the face. However, within a short time the fat disappears, so the procedure has little to recommend it at the present time. Perhaps in the future there will be improvements in the technique of fat transfer, so providing another feasible means of soft tissue filling.<br />
Cheek implantsWith age and weight loss the cheeks become hollow, allowing the skin to be pulled down by gravity. Silicone rubber cheek implants can be inserted to give the cheeks fullness. This also helps to suspend the skin, preventing the downward migration with gravity. Cheek implants are often inserted through the mouth in conjunction with face-lifting or during a resurfacing procedure. They are safe and cause few problems, although infection can occasionally be troublesome.<br />
Chin implantsChin implants are sometimes inserted in combination with nose surgery to balance the facial profile or in combination with face and neck liposuction to improve the angle between the chin and neck. Although many different materials are available, acrylic implants (designed by Dr Benito Rish) are the most natural implants available, feeling exactly the same as bone. Silicone implants are also often used because they are easier to insert, but have a less natural feel.Chin implants are inserted via a small cut under the chin, so infection is rare and the scar is rarely visible. Neck liposuction can be performed through the same incision.<br />

			<content:encoded><![CDATA[<p>All drugs used for medical therapy in the United States must first be reviewed and approved by the FDA. The FDA is responsible for assuring that all drugs, before they are sold, are sufficiently safe and effective. Accordingly, the FDA dictates that certain steps be followed from the time a drug is discovered until it is finally licensed. The process of testing drugs has several stages: a drug is tested first in the test tube and in animals, then in a few people for a short time, and then in a larger group of people for a longer time.     Preclinical trials-A new drug is first tested, not in people, but in test tubes and in animals. These first tests are called preclinical trials because they precede tests with people, which are called clinical trials. Preclinical trials determine the drug&#8217;s toxicity, its pharmacologic properties, and its effects against certain microbes. Of all the drugs tested in preclinical trials, only about one drug in a thousand is ever tested in people.     If a drug shows promise in the preclinical trials, the FDA grants it the status of an Investigational New Drug (IND). IND status must be granted before the FDA allows the drug to be tested in humans.     Phase one clinical trials. Phase one clinical trials are the first round of tests in humans. Phase one trials are designed to determine the safety and dosage of a new drug. These trials usually involve a relatively small number of people, usually between twenty and eighty. The drug is given only for a short time, usually for not more than a week or two, and sometimes only a single dose is given. Participation in a phase one trial may require staying in a research unit of the hospital, or the participant may visit an outpatient clinic which carefully monitors the trial. Participants in a phase one trial don&#8217;t benefit much when the trial is for an infection like HIV because the treatment lasts for such a short time. Therefore, people who participate in phase one trials are often paid for their services, or else they participate for entirely altruistic reasons.     Phase two clinical trials. Phase two clinical trials are the second round of tests in people.  Phase two trials usually involve one hundred to three hundred participants and last for months or years. The purpose of these trials is to determine the schedules for doses, to collect additional information on toxicity, and to test, at least preliminarily, the drug&#8217;s effectiveness. With HIV infection, the effectiveness of a drug may be determined by the effect on CD4 cell counts, by the numbers of HIV in the blood, by the delay in progression of disease as indicated by opportunistic infections or development of AIDS, and by how the participant feels. People often participate in phase two trials for access to the drug before licensing. Or if the trial is relatively short and benefits to the participant relatively brief, people participate for payment or altruism.     Phase three clinical trials. Phase three clinical trials are the last phase before the FDA licenses the drug. Phase three trials often include two thousand to three thousand or more participants and run for months or years. In this phase, the drug is given to participants in the doses and at the intervals that the FDA is likely to consider acceptable. The purpose of these trials is to get additional information about the safety and effectiveness of the drug. People usually participate in phase three trials for access to the drug before licensing.     NDA review. The next step is to take all the information gathered during the previous steps and submit it in a New Drug Application (NDA) to the FDA. The FDA reviews the NDA and decides whether to license the drug, what the dose should be, how the drug should be administered, which side effects require specific warnings in promotional material, and what conditions the drug is useful for. This and other information is available in the Physicians&#8217; Desk Reference, or PDR. The PDR is published each year and is available in most bookstores for $30 to $40. There is also an edition for nonprescription drugs, like ibuprofen, cold remedies, or Dramamine, called PDR for Nonprescription Drugs.     It should be noted that the FDA does not specify the exact conditions for which a physician can prescribe a drug. On the contrary, the FDA says that accepted medical practice often includes use of a drug for conditions not specifically approved in the labeling. Thus, licensing a drug means that the drug becomes available in pharmacies, and that physicians can prescribe the drug for virtually any condition.     Phase four trials. Phase four trials may be conducted after the drug is licensed in order to determine new dosing schedules, or to collect additional information about effectiveness or toxicity, or to compare the drug to other drugs. People participate in phase four trials of drugs, even though the drugs are already available in the marketplace, in order to have access to the best medical care and (sometimes) in order to receive free care.     Streamlining clinical trials-The process described above has established high standards for determining the safety and effectiveness of new drugs. Unfortunately, the process is also expensive and time-consuming. The average new drug in the United States costs $230 million to develop and requires an average of twelve years to move from the laboratory to licensing. Many people with HIV infection, however, have argued that they simply cannot wait twelve years for a potentially useful drug to make it to the marketplace.     In response to this argument, the FDA has streamlined the testing process for drugs that might be useful in treating HIV infection. The process now goes faster. Drugs for HIV infection get an expedited review: this means that the FDA examines the IND and the NDA more rapidly, and the phase three trials require fewer participants (around three hundred instead of the usual two thousand to three thousand). Moreover, drugs are made available earlier in the testing process through a mechanism called a Treatment IND. Under the Treatment IND classification, the drug can be prescribed by physicians other than the investigators between the time the trial is completed and the time the FDA reviews the complete NDA application.     A more recent attempt to streamline the process and make new drugs more quickly available is called the parallel program. The new drug may be prescribed to people with HIV infection who are not participants in the clinical trial during (and therefore parallel to) the phase three trials.     AZT was the first drug to be evaluated under some of the provisions of this streamlined process. The phase two trial began in February 1986; in September 1986, the first analysis of data showed the drug was clearly beneficial; the drug was then made available to people with AIDS through a Treatment IND, and the drug was licensed in March 1987, just thirteen months after the trial began.     Although the streamlined process provided access to the drug quickly, it must also be recognized that when AZT became available, the amount of information about it was limited. As a consequence, much of the work normally done in phase three testing was actually done during the phase four, post-licensing trials. These trials indicated that the original dose of the drug was probably too high and that the treatment was probably begun too late. Nevertheless, the experience with AZT showed the potential usefulness of streamlining clinical trials.*184\191\2*</p>

This is all understandable &#8211; and I&#8217;ve seen it dozens of times with my patients &#8211; but trust me when I say that it&#8217;ll   , all pay off in the end.</div>
<div id="_mcePaste">New products, particularly those with collagen-stimulating properties, need ample time to work; there&#8217;s no way that this can be accomplished overnight. I advise that people try out their new products for at least a month [two or three months is actually ideal, but I know how impatient we can all be) before moving on to the next thing. The fact is that you aren&#8217;t going to see any improvement in your skin after just a few days. Those products that exfoliate the surface of the skin will give you a faster result.</div>

<div id="_mcePaste">Many inside and outside the health sciences have wondered, &#8220;Why all the fuss about determining the origins of the AIDS pandemic and other outbreaks?&#8221; Resolution of where things come from guides decisions about our future, ultimately enabling us to put safeguards in place. Although there is a great deal of concern about the danger of emerging new diseases, the most important incident justifying this attention has been the AIDS pandemic. If it turns out that the AIDS pandemic resulted from polio vaccination, then we have no example from the twentieth century of a new kind of pandemic blindsiding society by arising from a secluded source. Nor do we have an example from the nineteenth century. The closest we can come are recurrences of known diseases such as influenza and cholera. To find a pandemic of a previously unrecognized disease caused by a previously secluded pathogen, we would have to go back to earlier centuries, to the first spread of exotic diseases such as yellow fever, cholera, and syphilis.</div>
<div id="_mcePaste">If, on the other hand, we can conclude that the AIDS pandemic was caused by a natural process rather than a medical bungle, then we will know that we should be ready for similar occurrences over the next few centuries. This recognition, however, should not distract us from the horrible infectious plagues that were here among us during the decades before the AIDS pandemic and that are still here. We have neglected them because we failed to recognize their infectious character. And we still do.</div>

<div id="_mcePaste">Screening test: 50 gm oral glucose load, with 1 hour value s 140mg/dl. The ADA recommends that the test be done between weeks 24 and 28 of pregnancy. The American College of Obstetrics and Gynecology (ACOG) recommends that all pregnant patients be screened and that the 1 -hour value of 130 (90% sensitivity) or 140 mg/dl (80% sensitivity) be used as the threshold for diagnostic testing. According to the ADA guidelines, testing is optional in low-risk groups: age &lt; 25 years, normal body weight, no family history of diabetes, and not a member of an ethnic/racial group with a high prevalence of diabetes (Hispanic, African-American, Asian, Native American).</div>

Conclusion:   HbA1 c is not recommended as a diagnostic test at present.</div>

<div id="_mcePaste">A new condition; pre-diabetes, was defined by the ADA in 2002. A person with IGT or impaired fasting glucose (1FG: fasting glucose of 110-125 mg/dl) is now defined as having pre-diabetes. Such people are at high risk for developing diabetes in the next decade and have an increased risk for coronary heart disease. The ADA has estimated that about 12 million people in the U.S. have pre-diabetes.</div>

The depressed person is encouraged to talk about feelings. The manic is frequently ushered off for a &#8216;vacation&#8217; on downers. The psychiatrist faced with , a patient claiming to be the Virgin Mary, broadcasting the wisdom of God in a downtown shopping center, is likely to first search for the right drugs to quiet the patient and appease the environment. The social worker endeavors to work with the client, his impossible family conditions, his neighbors, the police and even the court system. Many of these workers feel overburdened and impotent in the face of extreme states.</div>
<div id="_mcePaste">In spite of increasing sophistication in research and the interconnection between chemistry and psychotic behavior, there is still no one-to-one cause and effect relationship between disease agents and cures. The applicability of disease definitions based, to a great extent, upon the experiences of a given society seems to be a dangerous definition of a &#8216;physical&#8217; disease. A mentally healthy person in our culture is, according to medical definitions, capable of verbally relating thoughts and feelings in the absence of physical disease. Definitions in terms of given cultural norms are bound to have limited application since they are value judgements related to the observer&#8217;s psychology rather than empirical reports of the client&#8217;s individual language and body gestures. A qualitative classification can hardly be expected to produce quantitative changes; therefore, it is logically inconsistent to even search for quantitative causes for &#8216;disease&#8217; which have no quantitative measures!</div>

I know, every diet you have ever heard of touts the wonders of grapefruit But, contrary to public opinion, they have almost no fiber, so they slosh around in your stomach not doing any particular good. Plus, they have only a tiny number of active enzymes compared with other, far more interesting fruits. Eating citrus fruits is like putting a Band-Aid on a broken leg. They just don&#8217;t do much. The amount of vitamin C you derive is wildly overrated. The amount of vitamin A is negligible. And the bulk of those nutrients is concentrated in the seeds, the white membranous lining, and the skin. When did you last try eating grapefruit skin and orange seeds?</div>
<div id="_mcePaste">Admittedly, grapefruit is low-cal. But remember, when something is low-calorie it&#8217;s also low-energy. Put low energy in, and you get low energy out.</div>
<div id="_mcePaste">Lemons and Limes. Sad but true, these two are overrated. What I said about grapefruits and oranges goes for lemons and limes, too. They contain a special chemical that neutralizes the all-important pepsin enzyme which digests proteins. Eaten with or on a protein food—even the smallest squirt on a piece of fish or chicken—neutralizes the enzyme pepsin and renders the protein indigestible. Which means it&#8217;s fattening.</div>

Most people infected with these bacteria do not get pneumonia if their immune systems are normal. Pneumocystis carinii can be cured with medicines.</div>
<div id="_mcePaste">Exposure to Pneumocystis carinii is difficult to prevent and therefore medical treatment is recommended to prevent the infection. The best drug for preventing Pneumocystis carinii is trimethoprimsulfamethoxazole, a combination of two medicines. This medicine is recommended if the CD4 cell count falls below two hundred or if there are symptoms such as fever and fungal infection in the mouth (oral thrush).</div>
<div id="_mcePaste">Toxoplasmosis:</div>
<div id="_mcePaste">People with HIV infection usually have symptoms because of toxoplasmosis if their CD4 count is below hundred. This infection normally affects the central nervous system, including the brain. Many people infected with toxoplasmosis have no symptoms. However, people with HIV or</div>
<div id="_mcePaste">AIDS may have symptoms such as headache, confusion, and fever. Other symptoms include seizures, poor coordination, and nausea.</div>
<div id="_mcePaste">Toxoplasmosis can be spread in two ways: (a) by eating under-cooked meat; and (b) through contact with infected cat stool. It can be prevented by eating meat that is cooked till it is no longer pink in the centre. Red meat is safe if it has been frozen for at least twenty four hours. Chicken and eggs almost never contain toxoplasmosis. However, they should also be cooked until well done because of risk of other diseases.</div>
